CAMP FORMS

All required health forms must be completed and returned before the first day of camp. Forms will be distributed by the camp director via email. If you are a current CHS student, you will need an updated physical only if your current physical was completed more than 12 months prior to the first day of camp.

REFUNDS

If you need to un-enroll your child before March 1, you will recieve a full refund. Between March 1 - May 1, we will provide a 50% refund. After May 1, no refunds will be provided.

Photography Permission

I grant Concord Hill School permission to use my child’s photo in CHS publications
including both print and online (including CHS website and social media). Children will not be identified by name in any publication distributed outside the Concord Hill Community.
