If your child has allergies, asthma, seizures, or other medical issues that require acute care, please download, complete, and return the below forms to Patty Gallagher as soon as possible, and no later than JUNE 1, 2019.

REGISTRATION INFORMATION

PLEASE READ CAREFULLY...

Upon registration you will only be charged a deposit of $295 PER CHILD, which is required to secure your spot(s). The non-refundable deposit will be deducted from the total cost of the program. All camp fees must be paid in full by MAY 17, 2019. Invoices will be mailed in April. Cancellations made after May 17, 2019, will not be refunded. All required health forms must be completed and returned before the first day of camp.
